Information Requests/MN Governmental Data Practices Act Guidelines and Procedures

The City of Springfield is obligated to comply with The Minnesota Government Data Practices Act (MGDPA), which is a series of state laws that attempt to balance the public’s right to know what their government is doing, individuals’ right to privacy in government data created and maintained about them, and the government’s need to function responsibly and efficiently.

Notary Public

Having a document notarized means that your act of signing the document is witnessed by a notary public. The notary will ask you for identification to ensure that you are who you say you are and not an imposter. Notaries are not responsible for the accuracy of your application.

A notary is a responsible person licensed by the Minnesota Secretary of State to witness the signing of important documents. The current cost of a notary is $2.00 per signature. Please see the city hall hours above for notary services.

Birth Certificates

The Office of Vital Records maintains records for births occurring in the State of Minnesota back to 1900. If you want a certificate for a person born before 1900, contact the Local Issuance Office in the county where the birth occurred. The Office of Vital Records has registered births electronically since 2001.

If you need a certificate for someone born in another state, see the National Center for Health Statistics website for contact information.

  • You can get a birth certificate only if you have tangible interest
  • Some records confidential and can only be obtained by certain people.
  • Valid ID is required.
  • The fee is $26. If you order more than one copy in the same request, each additional copy of the same record is $19. Fees are non-refundable, as per Minnesota Statutes, section 144.226.
  • If no birth record can be found, you will receive a certified “Statement of No Birth Record Found.”

For same day service: You may visit the Springfield City Hall for records 1900-present. Pre-1900 records are available only in the county of birth. Applications are available there to complete.

For service by mail: Mail the completed birth certificate application for records 1900-present. The Birth Certificate Application must be signed and dated in front of a notary public. Payment is accepted by check or money order and is deposited upon receipt. Mail your application to: Attn: Birth Certificate, City of Springfield, 2 East Central St, Springfield, MN 56087.

Death Certificates

The Office of Vital Records maintains death records for the state of Minnesota. Deaths have been registered electronically since 1997.

State death records start in 1908. If you want a copy of a death record prior to 1997 please contact the local issuance office in the county where the death occurred. Death records from 1908-1997 are also available through the Minnesota Office of Vital Records.

If you want a copy of a record for a death that occurred in another state, see the National Center for Health Statistics website for contact information.

  • You can obtain a death certificate only if you have tangible interest.
  • Valid ID is required.
  • The fee is $13. If you order more than one copy in the same request, each additional copy of the same record is $6. Fees are non-refundable as per Minnesota Statutes, section 144.226.
  • If no death record can be found, you will receive a certified “Statement of No Death Record Found.”
  • Payment is accepted by check or money order and is deposited upon receipt.

For same day service: You may visit Springfield City Hall for Minnesota death records 1997-present. Pre-1997 records are available only in the county of death or through the Minnesota Office of Vital Records (see below). Pre-1908 records are available only in the county of death.
